The two official languages in Finland are Finnish and Swedish. Therefore there are no forms in English. Please, fill in either the Finnish language form or the Swedish language form. These instructions guide the customer through the filling in process.
In the upper right-hand corner the customer (the manufacturer, importer, exhibitor or distributor of the audiovisual programme) defines (with a check) whether the programme is registered (Kuvaohjelman ilmoituslomake / Anmälan av bildprogram) or its classification is applied for (Kuvaohjelman tarkastushakemus / Ansökan om granskning av bildprogram).
The customer fills in the information on the picture programme in the upper part of the form. The Finnish Board of Film Classification fills in the information in the lower part of the form and issues to the customer either an extract from the audiovisual programme register or the classification decision with instructions for appeal depending on whether the case involves a registration or a classification.
The register extract and the classification decision are official proof of the fact that the programme has been registered or classified as required by the law. The register extract or classification decision shall accompany each copy of the picture programme to be shown. The product packages of picture programme recordings rented, sold or otherwise conveyed for home use have to bear the age-limit and other identification information either on a sticker or in printed form. A separate notice contains information on the printing models and the ways for acquiring the stickers.
It is important to give the information requested on the form as complete as possible, especially when a registration of a picture programme is made.
Suomenkielinen nimi / Finsk titel = Finnish title
The title used of the programme in distribution in Finland.
Ruotsinkielinen nimi / Svensk titel = Swedish title
The Swedish title used of the programme in distribution in Finland.

If audiovisual program is distributed in Finland with bar code as program's idnetification nuber, shall this number be disclosed in the registration form. This doesn't apply to classified film.
Ohjaaja / Regissör = Director
Pääosissa / huvudrollesinnehavare = Main cast
This information is important because there may be several picture programmes with the same title and the names of the director and the actors will facilitate the identification of the programme.
Kesto/Pituus / Speltid/längd = Duration / length
The duration of the audiovisual programme in minutes or its length in metres (e.g. in the case of a 35 mm film copy). The information is important especially when making a notification when the Classification Board cannot itself measure the programme.
Aihe/Laji / ämne/genre = Topic / genre
Subgenre of a pornographic program (i.e. hetero/homo, SM)
Ohjelmaa esitetään ja levitetään vain 18-vuotta täyttäneille (ilmoitus) / Programmet visas och distribueras endast till personer över 18 år = The programme will be shown and distributed only to those over 18 years of age (registration).
If the audiovisual programme, due to its brutality or sexual content or by raising horror or in some other corresponding way is likely to have a harmful effect on the development of children, it may not be distributed to those under 18 years of age.

This point is filled in only if the audiovisual programme is brought in for classification. The customer checks the age limit that he hopes the programme will obtain in the classification.
Formaatti / Format = Format
The format in which the customer will show or distribute the picture programme.
Selostus elokuvan tapahtumista liitteenä (pakollinen) / Utredning om programmets handling = Account of the events of the programme (compulsory)
The form must always be accompanied by a short description of the plot (synopsis) or photocopies of covers of the program . The form will not be handled if the account is missing.
When pornographic programs are registered synopis is not needed if a copy of a cover of the video or DVD is attached to the form.
Palautettava aineisto noudetaan/ toimitetaan postitse Det material som återlämnas avhämtas/sänds per post = The material to be returned will be fetched/mailed
Information on how the material submitted to the Classification Board will be returned to the customer. This mainly applies to classification.
Ilmoittajan / tarkastuttajan yhteystiedot Kontaktuppgifter = Contact information
As complete contact information of the customer as possible: the name, postal address, telephone number, e-mail address etc. of the person handling the matter so that the customer can be contacted quickly if necessary.
Huomautuksia / Anmärkningar = Remarks
Here you can enter e.g. information on the premiere of the picture programme or other factors to be taken into account in the handling of the programme.

Paikka / Ort = Town
Allekirjoitus / Underskrift = Signature
Is necessary as the registration/application is not official if it is not signed.
